1. Acceptance of Terms

By opting in to the Salus Reflect SMS service ("Service"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, do not use the Service.

2. Description of Service

Salus Reflect provides an SMS-based wellness check-in service for student-athletes. The Service sends text messages to enrolled participants asking about their well-being after practices and games. Your responses help coaching and training staff better support you.

3. Eligibility

To use this Service, you must:

• Be a current student-athlete or authorized participant.
• Be at least 18 years of age, or have parental/guardian consent if under 18.
• Have a valid mobile phone number capable of sending and receiving SMS messages.
• Provide explicit consent to receive SMS messages.

4. Consent to Receive Messages

By opting in, you consent to receive recurring SMS wellness check-in messages from Salus. Message frequency varies but is typically 0-2 messages per day during your athletic season.

Message and data rates may apply. You are responsible for any charges from your mobile carrier.

5. Opting Out

You may opt out of the Service at any time by replying STOP to any message. After opting out, you will receive a confirmation message and no further messages will be sent unless you opt in again.

6. User Responsibilities

You agree to:

• Provide accurate and truthful information in your wellness responses.
• Keep your contact information up to date.
• Not use the Service for any unlawful purpose.
• Not attempt to interfere with or disrupt the Service.

7. Privacy

Your use of the Service is also governed by our Privacy Policy, which describes how we collect, use, and protect your information.

8. Disclaimer of Warranties

The Service is provided "as is" and "as available" without warranties of any kind, either express or implied. We do not guarantee that the Service will be uninterrupted, error-free, or secure.

The wellness check-ins are not a substitute for professional medical advice, diagnosis, or treatment. If you are experiencing a medical or mental health emergency, please contact emergency services immediately.

9. Limitation of Liability

To the fullest extent permitted by law, Salus and its affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Service.

10. Modifications to Service and Terms

We reserve the right to modify or discontinue the Service at any time without notice. We may also update these Terms from time to time. Continued use of the Service after changes constitutes acceptance of the modified Terms.

11. Termination

We may terminate or suspend your access to the Service at any time, with or without cause, with or without notice.

12. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Illinois, without regard to its conflict of law provisions.
